Dr. Jan Zackowski O.D.
Dr. Jan Zackowski grew up in the rural community of Creston, BC. Following graduation from Prince Charles Secondary School in 2001, Jan completed his Bachelor of Science degree with Distinction from the University of Lethbridge in 2005, and then his Doctor of Optometry degree with Distinction from Pacific University in Forest Grove, Oregon.
University training included clinical experience at the Ophthalmology Referral Centre in Atlanta, Georgia, the Veterans Hospital in Spokane, Washington, a public hospital in Portland, Oregon, and a private practice in Prince Rupert, BC.
Since graduation from optometry in 2009, Dr. Zackowski has practiced in the Kootenay communities of Creston, Nelson, Castlegar, Trail, Nakusp, and Kaslo as well as the North Coast communities of Terrace, Kitimat, and Smithers.
Dr. Zackowski is fortunate to work in conjunction with competent and experienced ophthalmologists in the area to best manage the needs of patients concerning cataracts, eye injury and trauma, glaucoma, and macular degeneration.
Outside the office, Jan enjoys playing sports and keeping an eye on the Blue Jays. He played baseball at a highly competitive level while attending the University of Lethbridge.
Optometry runs in the family, as Jan’s father, Dr. Gene Zackowski (University of Waterloo, Ontario, 1979) has owned Creston Optometric Eye Centre for thirty-five years. Both doctors are proud of their staff.
Dr. Jan Zackowski is a member of the BC Association of Optometrists and the Canadian Association of Optometrists.
Dr. Scott Foutz O.D.
Dr. Scott Foutz, originally from Oregon, completed his bachelor degree in molecular biology at Oregon State University. He went on to get his Doctorate of Optometry at Pacific University in Forest Grove, Oregon in 2009. Dr. Foutz continued his education by completing a residency at the University of Utah at the Moran Eye Center and the Veterans Administration Hospital. There he specialized in primary care, ocular disease, and specialty contact lenses.
Dr. Foutz worked at the University of New Mexico Hospital for the next six years. He worked in conjunction with the ophthalmologists at UNM and gained very valuable experience treating and managing ocular diseases, especially those associated with diabetes.
Dr. Foutz is passionate about optometry and helping others. He is a member of the BC Association of Optometrists and the Canadian Association of Optometrists. He donated his time and completed a medical mission trip to the Philippines helping those in need, and he plans to continue helping when he can.
Outside of work, Dr. Scott Foutz loves spending time with his wife and daughter. Together they enjoy skiing and exploring the outdoors. They are thrilled to be living in the beautiful Creston Valley.
